Sidney Dekker is professor and director of the Key
Centre for Ethics, Law, Justice and Governance at
Griffith University.

He is the author of several bestselling books on system failure, human error, ethics and governance, including most recently Patient Safety: A Human Factors Approach (CRC Press, 2011).
